This change is being done for several reasons. The first is that many candidates have been left, at the last second, by their family or friend who had something come up. “This is very common and almost always brings the student to tears,” said Kevin L. Miller, Executive Director of the State Board of Cosmetology. “It makes an already stressful situation worse by having to rely on someone else,” he added.
In addition, using a mannequin is much more cost effective for the student. Candidates can bring their mannequin head they have used all through school and are comfortable with. If students do not have a mannequin hand, you can make one to use for the manicure procedures.
The only stipulations are that all artificial heads and hands must not be pre-cut or have writing on it to give students guidelines or hints. If you are taking an exam after January 1, 2009 and need information on how to make a mannequin hand, visit the boards website at http://cos.ohio.gov for details.
With the renewal period nearly over, the board (at the time of this writing) has over 3,000 individuals who had their renewal applications returned to the Board as undeliverable. This is due to licensees relying on the post office to forward mail to their new address.
It is imperative that licensees remember it is their obligation to notify the Board of any address changes. Many of these individuals are in danger of having their license become lapsed, which will require them to re-take the state board exam to get their license back.

Q. How Can I Change my Name / Address?
A. A name change will require a copy of your legal documentation (marriage certificate, divorce decree, etc.) to be sent to the Board office. A certified copy is not required; a photocopy or scanned image is sufficient. You can send the information to the Board office by email, fax, or mail.
Address changes are much more common and even more important in order to receive your license, renewal application, etc. Address changes may be submitted to the Board office by phone, fax, email, or mail. Changes of Address orders that are filed with the U.S. Post Office expire after 12 months. There is still activity on an expired permanent Change of Address in months 13 through 18, during which the mail piece is returned and the sender is notified of the new address. After month 18, the mail piece is returned to the sender noting “Forwarding Order Expired”.
Q. How can I update my Board approved photo for my license?
A. You will need to submit one, recent photograph of yourself, approximately 2 ½ x 3 ½ in size. Uncut passport photos are recommended. Digitally produced pictures must be printed on photographic paper. Print your name, address and Ohio license ID# on the back of the picture and mail to the Board office for processing. Once processed, your photo will be stamped and returned by mail. Please note Polaroid pictures will NOT be accepted.
